The New Foundations Supervisor is responsible for supervising staff and coordinating housing services, collaborating with area providers, and ensuring continuity of care for New Foundations. The service is provided in an effort to provide stabilized housing, decrease voluntary hospitalizations, and ensure follow- up care.
The New Foundations Supervisor must demonstrate strong skills in establishing priorities, assigning and reviewing duties and tasks to staff, and resolving issues in the New Foundations Program. In order to manage multiple participants and staff, the Supervisor must demonstrate the ability to multi-task and delegate to achieve desired end results. The Supervisor should also demonstrate good communication skills, skillfully identify issues, and provide support to staff.
Responsibilities:
- Provide supervision and support to staff.
- Complete all documentation required for New Foundations according to the guidelines. Documentation is to be completed appropriately, legibly, and in a timely manner.
- Provide transportation for participants to/ from New Foundations when necessary and as assigned by the Director of Mental Health Programs.
- Review all New Foundations admissions, ensuring all criteria is met and orientation packets are complete.
- Be available via phone to support staff in the absence of the Director of Mental Health Programs.
- Work cooperatively with Behavioral Health & Developmental Services, Westmoreland Case Management and Supports, other mental health providers and other human service providers for housing planning.
- Coordinate all admissions to and discharges from New Foundations, including linkages with providers in the community. Consult with the Director of Mental Health Programs as needed, ensuring all admissions to New Foundations are appropriate.
- Schedule and conduct all discharge planning meetings for New Foundations.
Education, Skills and/or Experience
- Bachelor’s degree with major course work in either sociology, social work, psychology, gerontology, criminal justice, counseling, education or a related field
Or
- High School Diploma / GED and One (1) year previous casework experience – preferably in the mental health field.
And
- Direct experience in crisis intervention with the ability to assess crisis situations, respond by providing appropriate counseling, assist in resolving crisis situations and arranging for efficient referral and follow-up services.
- Knowledge of counseling skills.
- Must carry 100,000/300,000 bodily liability car insurance.
- Must complete a post job offer physical examination form in compliance with Chapter 5310.21 (e) of the Commonwealth of Pennsylvania Mental Health Manual. This form includes a screening for tuberculosis, is signed by a licensed physician and certifies the employee is free from any communicable diseases.

Salary: $46,758.00 – $51,168.00 per year
