Transitional Rehousing
The Transitional Housing program is a short term 3-6 months temporary housing program for Westmoreland County adults (18+) residents who are homeless with a mental health diagnosis or co-occurring disorder.
The program is intended to assist individuals with complex issues related to supports and the inability to maintain stable housing. Each participant receives an individualized housing plan to actively work on appropriate permanent housing. The transitional housing case worker collaborates with referral agencies and Westmoreland County area providers to ensure continuity of care. Participants have access to 24-hour crisis services.
