The Transitional Youth/Young Adults Program provides Case Management support to Juniors/Seniors in High School and to Freshmen/Sophomores in college, ages 17-21. Referrals will come directly from the schools/colleges. Through Case Management, we are able to connect the young adults to resources needed for their self-sufficiency goals. The program focuses on young adults who may not have the support systems in place in their home and/or personal lives. We will work with the individuals to identify their barriers and create a plan to aid them in their journey into adulthood.
